When Do You Need This Affidavit?

You need a lost document affidavit whenever an official document has been lost, stolen, damaged beyond recognition, or destroyed. The most common scenarios include:

Your South African ID book or Smart ID card has been lost or stolen - required by Home Affairs to issue a replacement.
